Tragic Interstate Incident: Life Lost After Two-Vehicle Accident in Washington County
A major highway accident on southbound Interstate 15 resulted in a fatality late Tuesday morning. State law enforcement officials confirmed that a multi-vehicle collision caused an SUV to veer off the asphalt and roll repeatedly on the side of the freeway.
The incident occurred on June 30, 2026, at roughly 10:46 a.m. near the mile 12 marker, located just outside Washington City limits.
Sequence of the Crash
According to the initial report released by the Utah Highway Patrol (UHP), a heavy-duty pickup truck and a sport utility vehicle were traveling side-by-side prior to the contact.
A Ford F-350 that was hauling a boat was moving along the center lane of traffic. Concurrently, a Toyota Land Cruiser occupied the right-hand lane next to the truck.

The Lane Shifting: The driver operating the pickup truck initiated a lane transition toward the right side of the freeway.

The Side Impact: As the truck moved over, its passenger-side door panels collided with the body of the adjacent Toyota.

The Loss of Control: The force of the contact caused the Toyota operator to lose steering control. The SUV careened off the right edge of the roadway and flipped over multiple times before finally coming to rest along the unpaved shoulder.
Emergency Response and Traffic Conditions
First responders arrived rapidly at the scene to extricate the female driver of the Toyota and deliver emergency medical care. Paramedics rushed her to a regional hospital facility; however, due to the extreme severity of the trauma she sustained, medical professionals later pronounced her deceased.
The occupants riding in the heavy-duty Ford truck did not suffer severe physical injuries during the encounter.
To preserve the scene for thorough examination and clear the heavily damaged vehicles, state troopers restricted traffic flow by shutting down the right-hand lane and shoulder for several hours. This closure caused significant backup delays for southbound commuters traveling through Washington County. The underlying factors behind the fatal I-15 rollover crash are still being reviewed as state investigators work to determine final fault.
